We take the confidentiality of our patient’s health information extremely seriously. Unless required by law, we will not release any details regarding your child’s health without consent.

Parents are responsible for accurate and up to date health card information. Please bring your health card for each visit. Failure to provide accurate and up to date health card information will result in your claim to OHIP being rejected.

If your baby is a newborn, the OHIP number from the hospital is only temporary. You must register with OHIP as soon as possible to obtain a valid permanent OHIP card.

If your child does not have OHIP coverage but has private insurance we will bill you for services and provide you with an official receipt. Use this receipt to claim reimbursement.

Due to OHIP regulations and privacy concerns, we are unable to offer any medical advice or management over the phone, fax or via email.
